One of my colleague was suffering the same recently. The antibiotic in combination with some pain killer shall start working within 1-2 hour. Sometimes it may take longer if the infection is also there. References :
I’ve had that ache with a UTI. It’s miserable! The antibiotics won’t directly help the pain, but they do clear up the infection. So when the infection is lessening, you’ll start to feel better. Even when you feel better, make sure that you finish the course of the antibiotics or the infection may return. Meanwhile, take tylenol or advil. If you’re not feeling better in about 3 days, you should call your doctor. Sometimes they need to change the type of antibiotic if the infection doesn’t respond to it. Hope you’re feeling better soon! References :

Antibiotics don’t do anything for the pain, they fight the infection. You may want to try some tylenol or advil for the pain.
